B What's Behind the Explosion & Use of Psychotropic Medication?

Invited Paper: The 1980s and 1990s were a period of rapid development of new drug treatments for mental illness. For many patients, the new classes of medications represent advances over existing treatments with respect to efficacy, side effects or both, but the diffusion of these medications has been accompanied by explosive increases in psychotropic expenditures. This session presents new research on several factors that influence the rapid increases in psychotropic drug spending, including direct-to-consumer advertisements, use of pharmacy management tools like prior authorization, expansion of drug coverage under Medicare Part D, and introduction of new product formulations of psychotropic drugs.

Methods for Measuring Health Disparities

Methods Workshop: This workshop will build on the ideas presented at the ARM 2006 and 2007 workshops on disparities measurement. Following a short overview of basic issues and concepts, presenters will explore issues in defining disparities elimination and real world applications of summary measures of disparity when dealing with ordered categories (such as income or education). We will also discuss continued advances in indirect methods for estimating race/ethnicity and disparities and use of GIS mapping tools to examine disparities at regional and local levels. Examples of use of these tools by selected plans participating in the National Health Plan Collaborative will be presented. Previous participation in either the 2006 or 2007 workshop would be beneficial but is not required.

Methods Workshop: This is an introductory session on hierarchical linear models, also called multi-level models and random effects models. The goal of this session is to understand the nature of clustering in data, why simple linear regression is inappropriate when clustering exists, and how to model clustering within data using fixed and random effects. Attendees should be familiar with simple linear regression.

Roundtable: Implementing and sustaining clinical process improvements often requires interventions not only with individual clinical microsystems that are the focal point for redesign but also with the larger organization of which the microsystem is part. This panel will discuss redesign from the project, organizational and inter-organizational perspectives. It will discuss evidence in the literature on effectiveness of redesign, and whether factors affecting effectiveness are contingent on the organizational context. It will also draw on the panelists' experience with redesign in both inpatient and ambulatory care settings.

Methods Workshops: When you read (or write!) an article concluding that quality improvement collaboratives or practice partnerships “worked”, or “didn't work” to improve quality of care, do you wonder what was done that worked or didn't work? If so, come discuss with our distinguished panelists how to understand which QI interventions were implemented, and how, in studies where interventions can be variable across sites. Our panelists are leading quality improvement researchers who have published about multi-site studies evaluating QI collaboratives and partnerships. Dr. Bruce Landon will discuss the challenge of measuring which QI strategies were implemented by sites in the Health Disparities Quality Improvement Collaboratives, so that we may understand better the how they influenced care process and outcomes. Dr. Elizabeth Yano will describe how multiple data sources produce a better picture of which care interventions were accomplished in VA depression collaborative studies. Finally, Dr. Lynne Nemeth will present a classification of QI strategies used to survey practices participating in the PPRNet A-TRIP study, a primary care quality improvement study, and that will be used in MS-TRIP, a study of medication safety in ambulatory care, to understand which QI strategies were adopted.

Special Session: Leaders from four HSR funding agencies that span three countries and include both public and foundation-based programs will compare and contrast their experiences in research governance. Following a brief description of the role of each funding organization in their respective countries, panelists will be asked to describe their approach to priority setting; how and when they use different funding mechanism such as investigator initiated proposals, topic-specific calls for proposal, contracts, etc.; whether they have established boundaries between research, program evaluation and quality improvement initiatives; how they promote the uptake of the research they fund and, lastly, whether and, if so, how they assess the impact of their own funding portfolios. Panelists and the audience will be invited to discuss the challenges and trade-offs implicit in each of these policy choices.

Methods Workshop: This session will present research examining new and controversial methodological issues related to cost-effectiveness analysis (CEA) as well as a novel application. The methodological issues covered will be cost estimation for CEA and subgroup analysis in CEA. Estimation of costs has long been a vexing issue because costs typically are very high for a small proportion of individuals. The most recent methods of cost estimation will be discussed in the context of how they best apply to CEA analysis. Because treatments that work for some individuals may not work for others, subgroup analysis for CEA is essential, yet the methods are still in their infancy. Research on a promising Bayesian approach will be presented. Application of cost-effectiveness analysis within health care has the potential to create a health care system that adopts interventions of higher value. The workshop will present how CEA can also be used to evaluate whether health care system reforms themselves promote higher value care. (Intermediate)
