As part of their efforts to identify the opportunities and challenges associated with linking payment reform to population health, AcademyHealth contracted with the Green and Health Homes Initiative (GHHI) to provide technical assistance to a large social service organization to explore structuring a risk-based contract with a major local Medicaid managed care plan to provide targeted services to a high cost-high needs population.
