Overview

This webinar will introduce Enhancing Systems of Care for Children with Medical Complexity (CMC) as a five-year Health Resources and Services Administration (HRSA) Maternal and Child Health Bureau (MCHB)-funded initiative. This 90-minute session will provide an overview of the initiative and its five Demonstration Sites working to improve models of care for CMC. It will also assess outcomes that are informed by and meaningful to CMC and their families, ensuring all CMC have the potential for optimal health and family well-being. The session will be framed by a parent caregiver, together with a provider perspective, who will address how to better engage patients to advance equity.
