Overview


This webinar focused on how to strengthen systems of care for children with medical complexity (CMC) and their families. Participants learned insights from the Environmental Scan Report, designed to guide Medicaid programs and partners in developing more responsive systems of care. The report highlighted field trends in designing flexible eligibility criteria, improved data collection methods, and integrating quality-of-life measures to better support CMC.

The expert panel, including leaders from Medicaid, Title V, and family advocacy, shared their perspectives on translating these findings into real-world action. Hear how they think about health home model definitions, how to address implementation challenges, and navigate the complexities of measuring care quality to reflect family priorities. By the end of the webinar, participants:

  • Learned the core definitional domains for Children with Medical Complexity (CMC) and Family-Centered Health Homes for CMC (FCHH CMC) and some operational challenges for implementing various core models.
  • Understood the quality measures currently available for uptake in Medicaid programs for assessing FCHH performance for CMC and related measurement gaps.
  • Discovered how select states are currently implementing FCHH or related enhanced care coordination program for CMC.
