Overview:  In July 2015, the Office of the National Coordinator for Health Information Technology (ONC) awarded AcademyHealth $2.2 million to lead the Community Health Peer Learning Program (CHP). Throughout this two-year program, AcademyHealth will work in partnership with ONC to establish a national peer learning collaborative for 15 competitively-awarded communities to address specified population health management challenges through increased sharing and use of data.  

Through the National Call for Applications, the CHP Program will competitively select 15 communities and make awards ranging from $50,000 to $100,000: ten participant communities will design Community Action Plans (CAPs) for improving community health through the expanded collection, exchange and use of data; and five Subject Matter Expert (SME) Communities will develop topical Learning Guides designed to provide guidance, and to facilitate sharing of population health lessons.

The CHP program call for applications is now closed. Thanks  to all who expressed interest in the CHP Program, and who submitted  applications. We look forward to the review process, and to launching the peer learning collaborative in January of 2016.

Alison Rein, M.S., is a senior director for Evidence Generation and Translation at AcademyHealth.  In this role, she directs several projects that investigate how new sources of data and expanded stakeholder engagement are helping to transform health, care and research.

Ms. Rein is the Project Director for the Community Health Peer Learning Program (CHP), and in that role will lead development of the learning collaborative and associated products designed to inform and enhance health IT enabled community based transformation. With support from the Commonwealth Fund and the Office of the National Coordinator for Health IT (ONC), Ms. Rein previously directed efforts to assist Beacon Community Cooperative Agreement Program participants in identifying, documenting, and disseminating the lessons and results of their individual initiatives. She also served as principal investigator on a PCORI contract that actively manages and facilitates collaboration among 50 pilot projects to advance the field of patient centered outcomes research (PCOR), and also oversees or contributes to other projects that focus on building infrastructure to support health system improvement, comparative effectiveness research, and PCOR.

Ms. Rein has expertise in health IT policy, stakeholder engagement, comparative effectiveness research, information management and governance, and community-based transformation, and has written and presented on a number of these topics. With nearly 20 years of experience in the health care field, she brings a range of skills to her work, including technical assistance design and execution; collaborative network development; research design; and policy analysis.
Prior to joining AcademyHealth, Ms. Rein was assistant director of food and health policy at the National Consumers League (NCL). She holds a master's degree in public policy analysis from the University of Rochester.

Beth Henry Johnson, M.P.H., is a senior manager for the Research and Education in HSR team. Ms. Johnson serves as the program manager for the Community Health Peer Learning Program, funded by ONC. Ms. Johnson also oversees online activities for an AHRQ-funded grant, the Electronic Data Methods (EDM) Forum. She also co-leads the Center for Diversity, Inclusion, and Minority Engagement at AcademyHealth.

Prior to joining AcademyHealth, Ms. Johnson worked as a health policy research fellow for the National Medical Association's W. Montague Cobb Health Institute.  During her tenure, she co-planned seminars on health disparities and cultural competency, and supported the planning and design of two online surveys - one focusing on the prevalence of cultural competency training and education among health care providers, and the other assessing provider access to health information technology.

Ms. Johnson holds a master's degree in public health from the University of Virginia and a B.A. in sociology and music from the University of Richmond.
