I hereby attest that I am fully cognizant of the risks involved in participating in the AcademyHealth Fun Run offered at the 2023 Health Datapalooza (the “Run”), including but not limited to risks brought about by physical exertion, and agree to abide by and adhere to the published safety rules of AcademyHealth and those of the Hyatt Regency Crystal City as applicable. I further attest that I do not suffer from any heart, lung or other medical condition or disease that might in any way hinder or prevent me from fully participating in the Run.
In consideration of my participation in the Run, I hereby release, discharge and hold harmless AcademyHealth, its officers, directors, employees, members, subsidiaries, agents, successors, and assigns, including, but not limited to, any vendor or third party working in conjunction with AcademyHealth to put on the Run, from any and all liability that may arise, directly or indirectly, now or in the future, by reason of any injury, damage, loss, or expense incurred in connection with my participation in the Run, including that caused solely or in part by the fault (including but not limited to negligence, gross negligence, and/or recklessness) of the above-named parties. This Release and Waiver of Liability shall be binding on my heirs, executors, administrators, successors, and assigns, and all minors under the age of eighteen (18) participating with me. I agree that if any portion of this Release and Waiver of Liability is found to be void or unenforceable, the remaining portions shall remain in full force and effect.
