In our current ecosystem, a tragic inefficiency exists. While research drives monumental progress, less than 20 percent of health innovations are widely adopted in clinical practice. Even for those that succeed, the journey from evidence to routine care notoriously spans an average of 17 years. This staggering delay leaves countless patients waiting on the sidelines for the very progress and solutions that science has already delivered. Applied implementation science is the essential playbook for accelerating the sustainable uptake of evidence-based innovation. AcademyHealth’s new Applied Implementation Science Fellowship will equip a generation of researchers with the indispensable skills and knowledge to catalyze this translation and adoption in real-world settings. Launching in 2026, with support from Amgen, this two-year program is designed to empower early- to mid-career embedded researchers ready to master and apply implementation science directly within their health system-based research programs.
Meeting the Moment: Why Applied Implementation Science Is the Key
At its core, implementation science (IS) is the disciplined pursuit of the methods and strategies that facilitate the timely and enduring uptake of evidence-based practices. Unlike traditional efficacy research, which tests new treatments under ideal conditions, IS dives into the "messy realities" of daily practice. It seeks to understand the critical factors—settings, populations, supports, and practical adaptations—that enable evidence to progress from academic journals into everyday bedside care. This pivot in focus, from what works (efficacy) to how to make it work sustainably (feasibility, acceptability, and maintenance), is what makes implementation science uniquely powerful in closing the evidence-to-practice gap.
IS has already yielded crucial tools and frameworks, but these cannot implement themselves. Scaling change and weaving evidence into the fabric of routine care demands a highly skilled workforce embedded within health systems. These professionals must be capable of translating complex IS methods into operational solutions that directly improve care for all communities.
The need to build this capability is paramount. The Applied Implementation Science Fellowship will not only instill a strong foundation in IS methodology but will also forge a cohort of researchers who are better equipped to champion evidence, drive its widespread adoption, and ensure that those improvements are sustained for the long term.
What Fellows Will Gain: Mentorship, Mastery, and Momentum
Fellows will spearhead their own applied implementation science projects within their care delivery systems, targeting critical areas like bone health (i.e., osteoporosis), severe uncontrolled asthma with or without associated chronic obstructive pulmonary disease (COPD), thyroid eye disease, and IgG4-related conditions—areas where translating proven evidence offers immediate, life-changing impact for patients. Throughout the two-year journey, fellows will gain advanced knowledge and skills to master the practical application of implementation science in the field.
