The applicant should be a state or local government agency, or a contractor working with a state or local agency (if the applicant is not a state or local agency, they must include a letter of support from the public agency they are working with as part of the user buy-in noted below).

The applications should include documentation of at least the following elements:

  • Problem statement: Definition of the specific goal or problem
  • Data specifications: Including data elements, sources, and standards
  • Interoperability: How the data model builds on national standards to ensure consistency of data across disparate systems
  • Visualization tools: The project includes a creative approach to make the data analysis results accessible and usable for both technical and non-technical audiences.
  • User Buy-In: Demonstration of support from key local stakeholders representing necessary perspectives
  • Results: Demonstration of successful testing and implementation in real world setting(s). The data analytics roadmap should be implemented at scale in a state, region or local area; a robust pilot test would be acceptable.

Preference will be given to DARs that focus on one of the following “use cases” that are aligned with national, state and local health innovation priorities:

  • Strengthening primary care infrastructure (e.g., use of primary care spending measures, characteristics and performance of physician focused APMs)
  • Addressing social determinants of health (e.g., multi-agency/sector/disciplinary/jurisdiction efforts to improve health of targeted groups)
  • Measuring and monitoring total costs of care (multi-payer, population based)
  • Reducing opioid and substance use (e.g., trends in mortality/morbidity, prescribing practices).
  • Building long term care services and supports in the community.
  • Supporting consumer engagement through transparency and other approaches?

The DAR should be open source or available in the public domain.  Proprietary models or solutions will not be considered.
