Evidence Roadmap: Care Coordination and Medicaid Eligibility for Formerly Incarcerated Individuals
In 2014, AcademyHealth conducted interviews with Medicaid policymakers and other experts to identify high-priority evidence and data needs relevant to Medicaid. This Roadmap responds to interviewees’ stated interest in evidence on coordinating care for justice-involved individuals.
This brief presents a selected set of research studies, systematic reviews, and other resources to help Medicaid policymakers and other decision makers assess existing evidence on strategies for coordinating care and facilitating Medicaid coverage for individuals after release from prison, jail, or other justice-related institutions. It is one of several Evidence Roadmaps that highlight existing evidence on high-priority topics identified by policymakers and delivery system leaders in AcademyHealth’s Listening Project reports.
