Community Research for Health Equity Program Theory of Change

The Community Research for Health Equity (CRHE) Theory of Change focuses on the collaborative efforts of CRHE and its grantees to communicate their intended outcomes, goals, and impact. It reflects the collective endeavors of Robert Wood Johnson Foundation, AcademyHealth, Design Impact, and CRHE grantees in supporting community-led research, aiming to reshape research practices and, ultimately, healthcare systems to enhance community health.

Everyone Counts. Everyone Should be Counted.

For the When All Are Counted project, a newly awarded grant under the Robert Wood Johnson Foundation's Community Research for Health Equity program, managed by AcademyHealth researchers are going to take a long, deep dive into how data is collected and shared in West Virginia to inform structural solutions to build a better, inclusive surveillance system in the state.
